Family Support Case Manager - Counseling

Description:
Provide linkage to resources, coordination of services and monitoring of progress to drive optimal family functioning. Conduct client assessments and provide on-site services at community agencies, schools, courts, or any other location where the client may need advocacy or assistance. Maintain case records in a timely and compliant manner. Document client's progress and interventions in accordance with HIPAA guidelines and other federal and state regulations. Provide advocacy for families in need of various resources. Have knowledge of available resources in the service area for children and families. Other reasonable duties as assigned. Education, Experience and Values: Bachelor's Degree in Human Service related field from an accredited academic institution At least 2 years' experience in casework or closely related work Excellent written and oral communication skills Experience working with youth from diverse populations and backgrounds Well organized with excellent time management skills Appreciates the cultural diversity of our community Bilingual (English/Spanish) a plus
