Job Description Waiver Support and Medical Transport Coordinator **All applicants must have a smart phone or tablet with internet access and location capabilities.** GENERAL STATEMENT: While this position specifies responsibilities, requirements and duties in certain operational areas, this person is an employee at will of Independent Living Services and will be expected to respond to such requests not specifically contained herein. This person must maintain the attitude that he/she works for the total organization and as such, exhibit proper cooperation, enthusiasm and interest in all areas deemed necessary for the continuing success of ILS programs. Knowledge of the ILS Policy and personnel Manual is required. JOB SUMMARY: The medical transport coordinator position provides transportation of consumers to and from medical appointments. The intent of a medical transport coordinator is to provide support to the Support Managers, The Assistant Program Directors, the Program Director, the Nurse and Executive Director. JOB DUTIES AND RESPONSIBILITIES : General duties and guiding principles: Focus first on the person being served, and understand that my role will require flexibility, creativity, and commitment. Know and respect the values of the people I support and facilitate their expression of choices related to those values. Provide advocacy when the individual's preferences, needs, or talents are neglected or overlooked. Encourage growth & recognize autonomy of individuals receiving services while being attentive reducing their risk of harm. Develop relationships with the individuals that are respectful, based on mutual trust, and t maintains professional boundaries. Help individuals I support understand and express their rights and responsibilities. Recognize that each individual has potential for lifelong learning and growth. Be conscious of my own values and how they influence my professional decisions. Maintain competency in my profession through continued learning and attending and being an active participant in staff meetings and trainings. Assume responsibility and accountability for my actions and decisions. Recognize the importance of modeling valued behaviors to co-workers, individuals receiving services, & community at large. Practice responsible work habits. Being on time for work Completing all necessary documentation to assure compliance with all state and federal regulations. These may include, but are not limited to: training records, personnel records, time records, incident reports, etc. Maintaining strict confidentiality for all consumer information, and facility operations JOB DUTIES AND RESPONSIBILITIES : Assist with medical appointments Offers support as needed to the Assistant Program Directors and Program Director Logs all scheduled appointments on ILS Google Calender Transports consumers inside and outside of the medical facility in a timely and punctual manner Completes medical forms during and after each appointment to ensure that the Support Manager and Nurse receive a copy Ensure that all medical forms are filed immediately in consumer files Transport medical equipment and supplies to consumers when needed Assist with consumer file management and keep file room and files organized. Assist Profiles Case Managers with obtaining medical paperwork form the medical clinics Establishes and maintains communication with Support Managers and ILS Nurse after medical appointments Respects and protects consumer confidentiality and hospital confidentiality by keeping all sensitive and medical information private Follows all protocols and procedures to ensure safe delivery of consumer, their possessions, supplies and laboratory specimens Completes medical filing as needed Auditing medical documentation at group homes and apartments ensuring that it meets licensure standards Aiding in vaccine clinics and documentation Organizing medical visit documentation and medication administration records (Monthly) Edits/updates MAR as required and prints monthly MAR for managers without access to printer (Monthly) Edits/updates medications on consumers' face sheets (Monthly) Assist Executive Director with special projects when needed Organize and mail consumer birthday cards monthly Be available to work with consumers in ILS facilities if needed This job description is not intended to be all-inclusive, and this employee will also perform other reasonable related job duties as assigned. The essential job duties require the Waiver Support and Medical Transport Coordinator to physically be present during work hours. The Waiver Support and Medical Transport Coordinator cannot telecommute or work from home. QUALIFICATIONS: High School Education or equivalent Ability to operate a vehicle and possess a valid AR driver's license in good standing Proof of vehicle liability insurance Ability to observe and record behavioral data Ability to communicate clearly with staff and consumers through use of telephone and other verbal means Must possess good listening skills Ability to lift at a minimum of 25 pounds Knowledge of independent living skills Experience and knowledge gained by working with people with developmental disabilities is helpful Supervisory and training skills Ability to evaluate and determine consumer's strengths and weaknesses BENEFITS: See employee Handbook - Section 201-201 A.
